Overview
A repeated-access (multi-dose) vial is intended to be entered by a needle more than once, drawing a portion of the contents on each occasion. Repeated access is convenient for reconstituted peptide solutions that will be used over several days or weeks, but it introduces risks not present in single-use vials.
Each needle entry challenges the vial's sterility barrier and mechanically stresses the stopper. Over many entries, the risk of introduced microorganisms and of stopper fragmentation compounds.
Why it matters
The bacteriostatic-water and storage-window guidance on compound pages assumes a repeated-access pattern. Understanding what "repeated access" costs makes the guidance more usable.
Key concepts
Needle discipline
Each access should use a fresh, sterile needle. Reusing a needle re-introduces skin flora and mechanical damage from the previous use into the vial.
Stopper disinfection
The stopper is disinfected with an alcohol swab before each access. This reduces the microbial load on the surface the needle crosses.
In-use limit as upper bound
The in-use window on the compound page or the practitioner reference is an upper bound, not a target. Discard early if there is any concern about handling or storage.
Common misconceptions
- "Bacteriostatic water makes repeated access safe." It reduces microbial growth; it does not make repeated access risk-free.
- "The vial is fine as long as it looks clear." Visual clarity is not evidence of sterility.
- "Once bacteriostatic, always bacteriostatic." The preservative is consumed over time and its effectiveness declines with repeated dilution from other diluents.
Practical interpretation
Where the practitioner reference documents a repeated-access pattern for a compound, the in-use limit and diluent choice are part of that pattern. Deviating from either changes the assumed safety profile.
Limitations
Compound-specific in-use limits vary. Do not extrapolate one compound's in-use window to another.
